Lending Library Member Participation Agreement























  • You can request two items at a time and they will be sent to you as soon as available. Your name is added to the list on a first come first serve basis so you may have a wait to get one or the another. Members must sign this agreement and return/e-mail this form to the OPAS Librarian before their request will be considered. Only members in good standing may participate in the OPAS Library. Rules: 1. Member agrees to contact the Librarian by e-mail the day they receive the DVD/ VHS in the mail. 2. Member may have possession of the DVD/VHS for two weeks, or less, and then they must mail it to the next member on the waiting list. The Librarian will provide you with the mailing info to the next person on the list. 3. Member assumes the cost of mailing the DVD/VHS to the next member. If there is no member on the waiting list, you must return the DVD/VHS to the OPAS Librarian. 4. Member assumes liability for any damages to the DVD/VHS while in their possession. 5. Member will notify by e-mail the Librarian the day they mail it to the next member on the waiting list or if they are returning it to the librarian. 6. Not all DVD copies will work on computers, but they will work in standard DVD players. Cleaning with a soft micro cloth & rubbing alcohol is a good idea.
